You could check with attorney. You might find one at an office supply store. Basicly it must state what the power of attorney covers and be specific as to the powers and list child's name and your name and be signed and notarized and dated. Or that is what I would think. This is not a legal opinion, but my guess. You might check with a local bar association to see if they have blanks to fill out. You might find one on the internet as well.
