It's not the physicians you need to worry about, it's the health care plan. As described, the procedure would be considered cosmetic and will probably not be covered by the plan. Call the Plan itself and ask whether the procedure would be covered. If not, there will be no physician that can do the procedure without getting paid cash by you.

No. Cosmetic surgery like that is NEVER covered by ANY kind of health insurance.
